Reviews show that problem handoffs were a contributing factor in 20-24% of malpractice claims in the outpatient and ED settings. A review of surgical errors found that 28% involved handoffs. An AHRQ survey found that 51% of hospital staff agreed that “important patient care information is often lost during shift changes.”
