SUMMARY:ED Grand Rounds: Acute Stroke Care – It’s a New Yea
DESCRIPTION:Prashanth Krishnamohan\, MDClinical Assistant ProfessorDepartment of Neurology & Neurological SciencesMedical Director of NeurologyStanford Healthcare ValleyCare\nPresented By:  Prashanth Krishnamohan\, MD \nThe American Heart Association released its most recent guidelines regarding the early management of Acute Ischemic Stroke at the International Stroke Conference in January 2018. The guidelines contain new recommendations on the evaluation and care of stroke patients in the emergency department. This presentation will review some of the new recommendations pertinent to the emergency department care of acute stroke patients\, the recent clinical trials that these recommendations were based on and how the recommendations might change the way we care for our acute stroke patients in the emergency department. \nDesired Outcomes: \n\n Identify the most recent guidelines on acute Stroke care\nState how the recent recommendations might change our current practices in the emergency department\nCite relevant evidence based literature to support the new recommendations\n\nCME Activity Flyer (PDF) \n

SUMMARY:Trauma Grand Rounds: Catheter Related Infections
DESCRIPTION:Daniel B. DiGiulio\, MDClinical Instructor\, Division of Infectious Diseases and Geographic Medicine\, Stanford University School of Medicine\nPresented By: Daniel B. DiGiulio\, MD \nCatheter-related infections cause a significant proportion of healthcare associated infections\, and are responsible for a substantial burden of disease in the US every year. Although urinary catheters and intravenous catheters might appear at first to have little in common with  each other\, both are artificial devices that can lead to iatrogenic complications\, particularly when catheters aren’t inserted and managed under optimal protocols. Various factors increase the risk of catheter-related infections\, including: 1) using invasive catheters when they’re not clearly indicated; 2) using suboptimal technique during their insertion and management; 3) leaving  catheters in place longer than necessary; and\, 4) failure to adequately monitor catheters for infectious complications. This presentation will discuss the problem of catheter-related infections\, and provide prevention and management strategies to minimize their occurrence and adverse health impact. \nDesired Outcomes: \n\nState the appropriate indications for urinary catheter insertion\nKnow the daily steps to assess the ongoing need for urinary catheter continuation\nDiscontinue urinary catheters per catheter removal protocols\nIdentify risks for Central Line Associated Bloodstream Infections (CLABSIs)\nUnderstand the need to perform daily assessment of the ongoing necessity for central lines\nDiscontinue central lines when no longer necessary\nArticulate the critical role of proper hand hygiene before and after all patient contact\, as well as the use of Maximal Sterile Barrier Precautions when inserting central lines\nRecognize patient populations that are at increased risk for outcome disparities due to catheter-related infections\n\nCME Activity Flyer (PDF)

SUMMARY:Medicine Grand Rounds: What the Science Says about Addiction
DESCRIPTION:What the Science Says about Addiction:   Neurobiological Aspects of Addiction and Medicines Used in Treatment \nMark Stanford\, PhD\, Director\, Medication-Assisted Treatment ProgramsJANUS of Santa Cruz\nPresented by: Mark Stanford\, PhD\, and Nancy Krauss\, PhD \nData from NIH indicates that about 20% of patients seen by family physicians have a substance abuse problem (excluding tobacco use). In the everyday practice of medicine it is important to consider substance abuse as part of the differential diagnosis. Substance abuse can lead to significant social and family dysfunction and cause problems with finances\, relationships and employment. Medical problems can be difficult to manage\, and chronic conditions can be exacerbated\, leading to increased morbidity and mortality. \n Desired Outcomes: \n\nIdentify the common neural pathways of drug addiction\nDiscover the mechanism of action of 2 medications in the treatment of opioid use disorder\nDiscover the mechanism of action of 2 anti-craving medications for alcohol use disorder\nIdentify 2 ways to refer patients into treatment for substance use treatment.\n\nCME Activity Flyer (PDF) \n

SUMMARY:ARU Grand Rounds: Dermatology for the Hospital Provider
DESCRIPTION:Amara Lieberman\, MDDermatologist\, Pacific Dermatology &Specialty Clinic\, NMC\nPresented by Amara Lieberman\, MD \nIn a hospital setting\, physicians are often faced with dermatology issues and need to identify those that require more intervention. A new study claims that one in four Americans are affected by a skin disease. The prevalence and cost of skin disease in the US are increasing and will continue to do so as the population ages. Physicians need to be knowledgeable about advances in the diagnosis and treatment of common dermatological diseases. \nDesired Outcomes: \n\nTreat common dermatological disorders seen in the hospital setting\nRecognize when to refer the patient to a dermatologist\nImplement a systematic approach to the diagnosis and management of common and uncommon skin diseases.\n\nCME Activity Flyer (PDF)

SUMMARY:NMC Grand Rounds: Emerging Advances in the Treatment of Aortic Stenosis: Transcatheter Aortic Valve Replacement
DESCRIPTION:Bialal M. Shafi\, MDCardiothoracic SurgeryPalo Alto Medical FoundationMedical Group\nPresented by:  Bialal M. Shafi\, MD \nTranscatheter aortic valve replacement (TAVR)\, sometimes referred to as transcatheter valve implantation (TAVI)\, was developed as an alternative for patients who require an aortic valve replacement for severe aortic stenosis\, but are not eligible for the conventional surgical aortic valve replacement (SAVR). Many patients diagnosed with severe aortic stenosis are considered too high risk for open heart surgery. As evidence continues to emerge\, the TAVR  procedure has now included moderate-risk surgical patients\, as well.  Careful patient selection\, methodical risk stratification\, optimal \nvalve sizing and a collaborative team approach with experienced surgeons and interventional cardiologists are important factors to achieve respectable outcomes. \nDesired Outcomes:   \n\nDiscuss rationale for Trans-catheter Aortic Valve Replacement (TAVR)\nReview data from seminal TAVR trials\nDelineate demographic utilization of TAVR\nDiscuss strategies to improve access to this technology\nRecognize the role of cardiac and vascular CTA in the planning process for structural heart disease\n\nCME Activity Flyer (PDF)
