SUMMARY:ARU Grand Rounds: Cancer Survivorship and Rehabilitation
DESCRIPTION:Arash Asher\, MD\, is the Director of Cancer Survivorship & Rehabilitation at the Samuel Oschin Comprehensive Cancer Institute at Cedars-Sinai.\nwith Arash Asher\, MD \nDr. Asher has a special interest in the physical and rehabilitative needs of cancer survivors who continue to experience the effects of their cancer or its treatment. Dr. Asher’s expertise is in the non-pharmacologic management of pain\, cancer-related fatigue\, cognitive dysfunction\, neuropathy\, exercise prescription for cancer patients\, and the management other musculoskeletal problems. \nDr. Asher completed a physical medicine and rehabilitation residency at the UCLA/Greater Los Angeles VA\, and a cancer rehabilitation fellowship at MD Anderson Cancer Center in Houston\, Texas. He is board certified by the American Board of Physical Medicine & Rehabilitation and Hospice and Palliative Medicine. \nDesired Outcomes: \n\nAppreciate the proposed pathophysiologic mechanisms behind cognitive dysfunction and cancer-related fatigue among cancer survivors;\nUnderstand mechanisms by which therapeutic exercise mitigates fatigue and possibly improves survival among cancer survivors;\nRecognize the different management principles between the negative and positive symptoms of peripheral neuropathy.\n\nCME Activity Flyer (PDF)

DESCRIPTION:Anastasia R. Klick\, MD\, MPH FM-NMC OB Fellow\npresented by Anastasia R. Klick\, MD\, MPH\, FM-OB Fellow \nThyroid disease in pregnancy has significant potential impacts in pregnancy\, for both mother and fetus. This grand rounds presentation will review the fundamentals of thyroid physiology and pathology in pregnancy\, and instruct participants in diagnosing and treating thyroid disease in pregnancy including hypothyroidism\, hyperthyroidism\, thyroid antibody positivity\, and postpartum thyroiditis.  \nDesired Outcomes: \n\nUnderstand the basics of physiology of thyroid hormones and thyroid disease in pregnancy\, and potential consequences of untreated hyper and hypothyroidism for mother and fetus.\nDefine hypothyroidism and hyperthyroidism in pregnancy based on TSH and T3/T4 lab results.\nIdentify indications for treatment of overt hypothyroidism\, subclinical hypothyroidism\, thyroid antibody positivity\, and hyperthyroidism in pregnancy.\nUnderstand fundamentals of medical treatment of thyroid disease in pregnancy including initial dosing\, timing of laboratory testing\, and adjustment of medications.\nIdentify and treat postpartum thyroiditis.\nIdentify appropriate situations in to refer patients to endocrinology.\nCME Activity Flyer (PDF)

SUMMARY:California End-of-Life Option Act
DESCRIPTION:Catherine S. Forest\, MD\, MPH\, Clinic Chief\, Stanford Health Care at Los Altos\, Assistant Clinical Professor of Medicine-Family Medicine\, Stanford University School of Medicine\nwith Catherine S. Forest\, MD\, MPH \nCalifornia recently became the fifth state to legalize assisted suicide. The new law\, called the End of Life Option Act\, will become effective January 1\, 2016. The law allows adults to receive and aid-in-dying drug from their physician if the physician determines they are suffering from a terminal disease. Physicians need to know the implications of this law\, and how to talk with and treat patients who are terminally ill. \nDesired Outcomes: \n\nIdentify the steps required by the patient and physician regarding requests for the aid-in-dying drug. \nDemonstrate the new forms that need to be completed and filed.\nDefine which vulnerable patient populations will not be able to request the aid-in-dying drug.\nEvaluate the physician’s right to participate and right to choose not to participate.\nRecognize cultural issues related to end-of-life care.\n\nCME Activity Flyer (PDF) \n

SUMMARY:Trauma Grand Rounds: The Not So Simple Ankle Fracture
DESCRIPTION:  \nJulius A. Bishop\, MD\, Associate Residency Program Director\, Stanford University School of Medicine\,\nAnkle fractures are among the most common injuries treated by orthopedic surgeons. While straightforward management will lead to successful outcomes in many cases\, some patients and some injuries are very challenging. This talk will address some of the difficult challenges in ankle fracture management and introduce strategies to optimize outcome. \nDr. Bishop specializes in treating fractures of the upper extremity\, lower extremity\, pelvis and acetabulum as well as the management of post-traumatic problems including malunion\, nonunion and infection. \nHe received his undergraduate and medical school degrees from Harvard University and went on to complete the Harvard Combined Orthopaedic Surgery Residency Program. He pursued his subspecialty training in Orthopaedic Traumatology at Harborview Medical Center in Seattle\, Washington. \nHis research interests include medical education\, studying clinical outcomes after musculoskeletal injury\, orthopaedic biomechanics\, the basic science of fracture healing\, and evaluating new strategies and techniques in fracture surgery. \nGap: \n\nThere are high rates of complications after surgical treatment of ankle fractures in geriatric and diabetic patients.\nThe best technique for reducing and stabilizing the ankle syndesmosis is unknown.\nThe appropriate indications and best techniques for posterior malleolus fracture treatment are controversial.\n\nDesired Outcomes: \n\nUnderstand how to optimize surgical treatment of ankle fracture in poor hosts.\nIdentify the best way to reduce and stabilize the syndesmosis.\nEstablish indications and techniques for posterior malleolus surgery.\n\nCME Activity Flyer (PDF)

SUMMARY:Cardiology Case Conference
DESCRIPTION:M. Kerala Serio\, M.D. FACP\, FACC\, Cardiology Service Director\, NMC; Associate Clinical Professor of Medicine\, UCSF\nwith M. Kerala Serio\, MD \n3 Different Patients with Dual Antiplatelet Therapy Requiring Non-Cardiac Surgery \nDual antiplatelet therapy\, DAPT\, is prescribed to millions of patients worldwide following coronary artery stenting. A significant number of these patients undergo non-cardiac surgery. This poses a significant dilemma because of the risk of stent thrombosis and myocardial infarct versus the risk of perioperative bleeding. 3 patients will be presented with 3 different common clinical scenarios\, leading to evidence-based discussion of practical strategies for managing perioperative antiplatelet therapy. \nCME Activity Flyer (PDF)
