DiGiulio\, MD \nCatheter-related infections cause a significant proportion of healthcare associated infections\, and are responsible for a substantial burden of disease in the US every year. Although urinary catheters and intravenous catheters might appear at first to have little in common with  each other\, both are artificial devices that can lead to iatrogenic complications\, particularly when catheters aren’t inserted and managed under optimal protocols. Various factors increase the risk of catheter-related infections\, including: 1) using invasive catheters when they’re not clearly indicated; 2) using suboptimal technique during their insertion and management; 3) leaving  catheters in place longer than necessary; and\, 4) failure to adequately monitor catheters for infectious complications. Many patients diagnosed with severe aortic stenosis are considered too high risk for open heart surgery. Health professionals often assess capacity differently in practice. Provided their patients have some understanding of their illness and have some plans for meeting basic needs\, psychiatrists are often inclined to give patients the freedom to refuse care even if they do not exhibit a full understanding of the medical facts of their case and why they are refusing it. Adult medicine physicians\, in contrast\, are inclined to require patients to state a more complete understanding of the benefits and burdens of evaluation and treatment before allowing them to refuse care when their refusals might result in adverse medical outcomes. DESCRIPTION:Christina Maslach\, PhD.\, Professor of the Graduate School Professor of Psychology\, UC Berkeley Medical School\nSpeaker for this CME Activity:  Christina Maslach\, PhD \nChristina Maslach is a Professor of Psychology (Emerita) and a researcher at the Healthy Workplaces Center at the University of California\, Berkeley. She received her A.B. from Harvard\, and her Ph.D. from Stanford. She is widely recognized as one of the pioneering researchers on job burnout\, who has written numerous articles and books\, including The Truth About Burnout\, and has developed the leading research measure (the Maslach Burnout Inventory).  Several of her articles have received awards for their significance and high impact\, including her longitudinal research on early burnout predictors\, which was honored in 2012 as one of the 50 most outstanding articles published by the top 300 management journals in the world. Recently\, she received the 2017 Application of Personality and Social Psychology Award\, as well as a lifetime career achievement award for her work on burnout. Christina received national recognition as Professor of the Year by the Carnegie Foundation and The Council for the Advancement and Support of Education.  She has been president of the Western Psychological Association\, is a fellow of the American Association for the Advancement of Science and of the European Academy of Occupational Health Psychology\, and has received the Berkeley Citation and the Distinguished Teaching Award from U.C. Berkeley. \nOverview: \nThis lecture will provide a summary of what the burnout experience entails\, and its causes and consequences. She will be hypotensive due to both hypovolemic shock from a pelvic fracture and obstructive shock from a tension pneumothorax. DESCRIPTION:Bill Morris\, MD\, Medical Director – MAT Services\, JANUS of Santa Cruz\nSpeaker for this CME Activity:  Bill Morris\, MD \nGiven that chronic opioid therapy may benefit some patients with chronic pain\, but that the risks of chronic opioid therapy appears to outweigh the benefits in a large proportion of patients a crucial clinical skill is the ability to safely utilize chronic opioid therapy for the group patients in chronic pain in which opioids remain effective while at the same time recognize and manage those patients for whom chronic opioid therapy has failed. \n This presentation will review a proposed clinical definition of “opioid failure” developed by the SafeRx Santa Cruz Opioid Safety Coalition. DESCRIPTION:OB Fellow\, Natividad Medical Center\nPresented by:  Rebecca Raymond\, MD \nWith California’s recent legalization of recreational cannabinoid usage the rates of use are expected to increase. This includes increase in self-administered usage during the prenatal and postpartum periods for many women seeking to control hyperemesis and pain. Studies have shown that although legalization may not increase the baseline rate of usage in the general population it can have an effect on the level of exposure of recreational users with some studies indicating more than two fold increases in concentration of cannabinoids in tested meconium samples. Increased usage and availability of cannabinoids may have significant impact on rates of preterm deliveries\, low birth weight and may have longstanding negative neurodevelopmental effects on drug exposed fetuses.
