SUMMARY:OB Fellows Grand Rounds: Marijuana in Pregnancy: Legalization and Effects on Care
DESCRIPTION:OB Fellow\, Natividad Medical Center\nPresented by:  Rebecca Raymond\, MD \nWith California’s recent legalization of recreational cannabinoid usage the rates of use are expected to increase. This includes increase in self-administered usage during the prenatal and postpartum periods for many women seeking to control hyperemesis and pain. Studies have shown that although legalization may not increase the baseline rate of usage in the general population it can have an effect on the level of exposure of recreational users with some studies indicating more than two fold increases in concentration of cannabinoids in tested meconium samples. Increased usage and availability of cannabinoids may have significant impact on rates of preterm deliveries\, low birth weight and may have longstanding negative neurodevelopmental effects on drug exposed fetuses. Providers must be prepared with evidence to screen\, counsel\, and manage patients with cannabinoid usage before\, during and after pregnancy. \nDesired Outcomes: \n\nUnderstand of current legalization of cannabinoids in California and in the United States\nUnderstand of known effects of cannabinoid usage as well as differences in mode of usage during pregnancy and lactation.\nExploration of legal ramifications of Usage in the prenatal and postpartum periods.\nExploration of the rates of usage of cannabinoids and concurrent use of other substances\nUtilize Screening tools in practice\n\nCME Activity Flyer (PDF)

He is expert in laparoscopic and robotic surgery.\npresented by:  James Lilja\, MD \, Gynecologic Oncology\, new to Natividad Medical Center \n\n  \nOvarian cancer is a disease with a poor prognosis and limited options in terms of screening\, diagnosis\, and treatment. Ovarian cancer is the second most common type of gynecologic malignancy and the most common cause of death from gynecologic cancer. It is the fifth most common cause of cancer deaths in women. In the United States and Canada every year\, over 25\,000 women are newly diagnosed with ovarian cancer\, and 16\,000 women die from the disease. \nDesired Outcomes: \n\nDiscuss the benefits of B salpingectomy –may prevent cancer\nIdentify when salpingectomy should be considered (after completion of childbearing) at the time of hysterectomy\, in lieu of tubal ligation\, and also at the time of other pelvic surgery.\n\nCME Activity Flyer (PDF)

Klick\, MD\, MPH\, FM-OB Fellow \nThyroid disease in pregnancy has significant potential impacts in pregnancy\, for both mother and fetus. This grand rounds presentation will review the fundamentals of thyroid physiology and pathology in pregnancy\, and instruct participants in diagnosing and treating thyroid disease in pregnancy including hypothyroidism\, hyperthyroidism\, thyroid antibody positivity\, and postpartum thyroiditis.  \nDesired Outcomes:  \n\nUnderstand the basics of physiology of thyroid hormones and thyroid disease in pregnancy\, and potential consequences of untreated hyper and hypothyroidism for mother and fetus.\nDefine hypothyroidism and hyperthyroidism in pregnancy based on TSH and T3/T4 lab results.\nIdentify indications for treatment of overt hypothyroidism\, subclinical hypothyroidism\, thyroid antibody positivity\, and hyperthyroidism in pregnancy.\nUnderstand fundamentals of medical treatment of thyroid disease in pregnancy including initial dosing\, timing of laboratory testing\, and adjustment of medications.\nIdentify and treat postpartum thyroiditis.\nIdentify appropriate situations in to refer patients to endocrinology.\nCME Activity Flyer (PDF)
